Chronic pulmonary infection with several bacterial species including Pseudomonas aeruginosa is a hallmark of the most common, among Caucasians, life-shortening genetic disease, cystic fibrosis (CF). Resistant strains of bacteria have complicated therapy prompting development of novel antimicrobials. A series of silver-based antimicrobials, silver N-heterocyclic carbene complexes, inhibit the growth of all bacterial species tested to date including the CF lung pathogens P. aeruginosa and Burkholderia cepacia complex organisms. Silver carbene complexes (SCCs) are water soluble, and hence amenable to nebulization for drug delivery and many complexes can be incorporated into nanoparticles for nebulized delivery of long-acting, broad-spectrum antibiotics. SCC-loaded nanoparticles of a variety of formulations have demonstrated efficacy both in vitro and in mouse infection models and may ultimately provide a novel therapeutic approach to the CF patient.
